While there is currently no standard protocol in place for when to initiate EEN in adults with Crohns, there are small studies and case reports promoting its efficacy in certain complicated cases, Leber notes.
For example, a case series published in 2019 in Crohns & Colitis 360, a journal of the Crohns and Colitis Foundation, looks at the use of EEN in 13 adult patients with complex Crohns disease. The participants were offered EEN based on either meeting malnutrition criteria, inability to receive immunosuppressive therapy, or Crohns disease that was resistant to multiple therapies. Many also had complications such as abscesses or fistulas. Participants received EEN for two or more weeks with outcome data recorded at two and six weeks. The report found that 9 of the 13 patients experienced subjective improvement in well-being, and the authors concluded that EEN can help lead to healthy weight gain and reduce the need for surgery in people with Crohns.
Cavagnaro notes that patients with small bowel Crohns disease who are currently in a flare may also benefit from EEN as a bridge to getting on medication to help the flare. Starting EEN may help with symptoms and get the patient on the road to remission more quickly while waiting for a biologic medication to start to work, which can take time, she says.

1. Applesauce: Since your gastrointestinalsystem is experiencing a lot of irritation during a flare, you may want to stick to soft, easily digestible foods like applesauce. Be sure to choose an unsweetened variety though, because added sugar can cause more inflammation. You can also make your own sugar-free applesauce by cooking peeled, sliced apples with some water and then pureeing the mixture.

Management Of Patients With Abscesses And Fistulas
There are no randomized controlled trials or prospective observational data on the type of feeding or the benefits of bowel rest in patients with abscesses or fistulas. Often patients are given PN in order to allow bowel rest, but there is scant literature to support this practice. The first step of management should always be to control infections. Once an abscess has been drained and it is established that there is no free perforation or need for emergent surgery, choice of nutrition is paramount. While PN and bowel rest have been shown to result in high rates of spontaneous closure of enterocutaneous fistulas , it has not been proven to reduce overall mortality in patients with an ECF, possibly due to septic complications. Management of central lines and the rate of line infections have appreciably changed since many of these studies were conducted, so the risk to benefit balance of PN for patients with ECF may be more favorable today. Nonetheless, many patients may be able to tolerate oral nutrition following drainage of all abscess collections. Patients with high output ECFs may need PN for both nutrition and to help maintain fluid and electrolyte balance.

Parenteral Feeding In Cd
TPN allows bowel rest while supplying adequate calorific intake and essential nutrients, and removes antigenic mucosal stimuli. However, TPN is expensive, invasive and has a number of side effects. TPN has been shown to bring about remission in CD. Müller et al have found that, in 30 consecutive complicated CD patients, 3 wk of TPN as an inpatient followed by an additional 9 wk at home, during which time, no medication or oral intake was allowed, 25 patients avoided surgery. These patients returned to work and needed no further medication and ate normal meals subsequently. In a prospective randomized controlled trial , 51 patients with active CD refractory to medical treatment were treated with TPN and nil by mouth, defined formula diet via a nasogastric tube, or partial parenteral nutrition. Clinical remission was obtained in 71% of the patients on TPN, 58% on enteral feeding, and 60% on partial parenteral feed.
