There are several reasons why people with IBD may be at risk for malnutrition. The following list includes some side effects that contribute to malnutrition.
- Inadequate food/fluid intake may by caused by nausea, abdominal pain, loss of appetite or altered taste sensation
- Increased losses â intestinal inflammation during acute flares results in increased protein losses, losses from fistula fluids, diarrhea and bleeding
- Increased nutritional needs â inflammation or infection increases metabolic requirements
- Malabsorption with Crohns disease may be caused by severe intestinal inflammation, resection of small intestine and medications, such as prednisone and sulfasalazine
What About Vitamins And Minerals
It is a good idea for all people with IBD to take a standard multivitamin every day. People with IBD who are doing well do not need any extra vitamins or minerals. If you have Crohns disease is in the ileum or the ileum has been removed you may need to take B12, calcium, or vitamin D. You may need iron supplements if you have blood loss during inflammation or reduced iron absorption as a result of inflammation. Diarrhea or vomiting can cause loss of potassium and magnesium. If you are lactose intolerant and avoid dairy products, this can lead to low calcium. Ask your doctor if you need to be tested for any of the above.

How Would Observing These Guidelines Help
The recommendations were developed with the aim of reducing symptoms and inflammation. The ways in which altering the intake of particular foods may trigger or reduce inflammation are quite diverse, and the mechanisms are better understood for certain foods than others.
For example, fruits and vegetables are generally higher in fiber, which is fermented by bacterial enzymes within the colon. This fermentation produces short-chain fatty acids that provide beneficial effects to the cells lining the colon. Patients with active IBD have been observed to have decreased SCFAs, so increasing the intake of plant-based fiber may work, in part, by boosting the production of SCFAs.
However, it is important to note disease-specific considerations that might be relevant to your particular situation. For example, about one-third of Crohns disease patients will develop an area of intestinal narrowing, called a stricture, within the first 10 years of diagnosis. Insoluble fiber can worsen symptoms and, in some cases, lead to intestinal blockage if a stricture is present. So, while increasing consumption of fruits and vegetable is generally beneficial for Crohns disease, patients with a stricture should limit their intake of insoluble fiber.
What About If Im Avoiding Fibre Or On The Low Residue Diet
Many people avoid vegetables if their gut is flaring, but Ive chosen sweet potato and carrot as when peeled and cooked they contain mostly soluble fibre.
Soluble fibre isnt the tough, fibrous type-it instead gently helps your colon recover from both constipation and diarrhoea-so its not a laxative in any way but the blending will also break down any fibre either way-so itll be much easy to digest!
Well cooked carrots and sweet potato are allowed on the low residue diet but if in doubt, you could just double up on potato

Eating For Ulcerative Colitis The Big Picture
At first glance, it seems there are thousands of diets.
Every day a new diet and a new theory and a new pundit with another book to sell seems to appear.
Truthfully, there are only a handful of actual diets.
Everything else is just a variation on a theme.
There are vegetarian variations including vegan and raw vegan, there are all the Paleo variations like Paleo, Keto, Primal Blueprint, Carnivore, etc.
Those are like opposite ends of the spectrum. Sort of.
In between are diets that include various grains or that eliminate certain meats and not others. Eating fish, eggs and cheese but not beef or pork, for example. Or avoiding wheat while including supposedly safe grains.
Know Your Trigger Foods
Some fruits and vegetables create more gas in our GI tract, says Manning, like broccoli and cabbage, as well as watermelon and cherries, so I advise against eating these when symptoms are severe. Instead, says Julia Zumpano, a registered dietitian at the Cleveland Clinic, try cooked carrots, peas, or green beans, or peeled white or sweet potatoes, squash, or cucumbers because these vegetables are usually well tolerated by people with ulcerative colitis. Fruits that you might more easily digest include bananas, cantaloupe, honeydew melon, papaya, mango, and apples that have been peeled and cooked into homemade applesauce.
Keep in mind, however, that foods can affect people differently. The fruits and vegetables mentioned here are only suggestions, and youll need to determine your personal triggers through trial and error. Many people are unsure what causes , says Manning, so they avoid whole categories of food unnecessarily. When this happens, nutrition can become compromised and deficiencies can occur. Keeping a food journal and working with a registered dietitian can help you detect patterns and identify troublesome foods.

Eliminating the lettuce from this traditional Nicoise salad allows people with inflammatory bowel disease to enjoy a salad without the roughage, says Webb. Roughage refers to high-fiber foods that are generally less tolerable with Crohns or colitis.

Modify Diets To Include Soft Fiber And Cooked Veggies
Warren tends to recommend anti-inflammatory diets rather than individual foods. The Mediterranean diet, for example, includes a wide variety of fresh fruits, vegetables, fish, nuts, and healthy fats such as olive oil. Warren recommends modifying the basis of this diet to incorporate these foods in a more digestible form. For example, she suggests opting for almond butter instead of whole almonds, cooking green leafy vegetables instead of eating a raw salad, and choosing inherently soft forms of fiber, such as avocado, sweet potato, and papaya.
When cooking vegetables, try steaming or microwaving instead of boiling to avoid losing nutrients during the cooking process. Warren also suggests pureeing vegetables to make nutritious soups and smoothies that are easier to digest than whole produce.
During a flare-up, Kennedy says its best to stick to foods that are easy to digest, such as refined grains , and low-fiber fruits such as bananas, cantaloupe, and honeydew melon. Removing the peel from other fruit, like apples, also cuts back on irritation-causing fiber.
Kennedy says to incorporate noncruciferous vegetables, including bell peppers, squash, eggplant, tomato, and zucchini, which have low fiber content and are easier to digest. Cruciferous vegetables, such as broccoli and cauliflower, on the other hand, have high fiber content and can be irritating. Avoid dishes that include these ingredients and always avoid spicy foods, says Kennedy.

Do Certain Desserts Cause Flares
Diets high in sugar and fat have been associated in multiple studies with worsening inflammation. These ingredients are commonly present in trigger foods, which are items that can make your ulcerative colitis symptoms worse and cause a flare.
One study of people with ulcerative colitis found that eliminating these foods helped people experience symptom remission:
- High-fructose corn syrup
- Packaged or convenience foods
Dairy products have also been shown to make ulcerative colitis worse, so a lactose-free diet may be beneficial.
Many desserts do include these ingredients, so its important to know what causes your flares so you can avoid them when you have dessert.
What To Eat During An Ulcerative Colitis Flare Up
When youre in a flare, the most important thing to do is follow your doctors advice and avoid high fibre, irritating foods. For my clients who are already plant-based, this can take some work to lessen fibre while maintaining your nutrient intake. You may need doctor-supervised bowel rest, where nutrition is provided via other means. Or, you may need a low residue diet until the flare subsides.
However, you may also be advised to simply eat whatever you can if this is the case, I find is that so often, the foods chosen are not very healthy and may further the inflammatory response. So here, I recommend juicing small amounts of fruits and vegetables, and enjoying a lot of blended foods like smoothies, blended soups and easy to digest foods like noodles, nut butters, plant-based yogurts and rice.

Fruit And Hemp Protein Smoothie
This particular smoothie recipe has been a daily staple for me for many years now:

A Caveat on the Psyllium Husk Fiber
If youre not well-healed or are in a flare-up, the psyllium husk fiber will likely be too much for you. Dont use that ingredient until youre more healed and your digestive tract is more robust.
A Caveat on the Ground Flax Seeds
This one is kind of a rule bend. Flax seeds are not allowed on a strictly Paleo-AIP eating template.
Ive personally been using them forever and my immune system is fine with them. I feel the additional fiber is quite beneficial for me.
But Ive tested this in my own body extensively. This is an empirical thing.
Not everyone can handle them.

A Glass Of Cabbage Juice A Day Keeps The Ulcers Away
Cabbage is a delightful cruciferous vegetable but chances are you are more familiar with its siblings broccoli and cauliflower. Unfortunately cabbage doesnt get a lot of press or attention in the United States unlike many other countrieswhen I lived in Costa Rica it didnt seem matter what time of day there was always a cabbage salad included in the meal.
So why should cabbage get more attention? Cabbage contains the amino acid glutamine along with the anti-inflammatory glucosinolates which help heal an inflamed digestive tract. Hummus is made with mashed chickpeas, olive oil, and other seasonings.
The Crohns and Colitis Foundation recommend 1/4 cup of hummus and 2 ounces of pita chips as a snack.
To make hummus at home, a person can blend chickpeas, tahini, and a little olive oil together. They can add seasoning to taste but should avoid adding too much salt.
Even though it is high in fiber, pita chips and hummus can be a generally well-tolerated snack or small meal. The chickpeas in hummus may help symptoms in a person experiencing gas or bloating.

Protein is one of the key nutrients that someone with UC needs. For this grab-and-go snack, simply add a few cooked veggies to the bottom of a muffin tin and top with whisked eggs.
