Ulcer In Urinary Bladder

Hunner’s ulcers, simply put are painful ulcers in your urinary bladder. These ulcers are inextricably linked to another disorder called the painful bladder syndrome or intestinal cystitis. They occur in 10 to 15 percent of people with interstitia cystitis. These, in fact, are red patches or lesions on the bladder wall which can stiffen tissue and cause reduced bladder capacity. Hunners ulcers bleed, ooze pus and can be of different sizes. The ulcers in the bladder can be extremely painful and uncomfortable.

These are called Hunners ulcers because they were first described by Dr. Guy LeRoy Hunner, a Johns Hopkins gynecologist, in 1915. Since Hunners ulcers are seen only in people with intestinal cystitis, its important to know more about this painful life-altering bladder disorder before we can even understand how to deal with them.

How Are Peptic Ulcers Treated

Most H. pylori-related ulcers can be cured. Treatment involves taking two kinds of antibiotics for 2 weeks and an acid suppressor for 2 months or longer. The ulcer may take 8 weeks to heal, but the pain usually goes away after a few days or a week.

A child with an H. pylori-related ulcer should take all the antibiotics as directed by the doctor. Even if the symptoms stop, the infection may not be gone until your child takes all the medicine. Also, be sure to give your child the prescribed acid-reducing medicine.

To be sure the treatment worked, doctors may order a stool test to see if the H. pylori bacteria are gone. If symptoms last or get worse, doctors might do a follow-up endoscopy in 6 to 12 months to check for H. pylori.

Ulcers from NSAID use rarely need surgery and usually get better with an acid suppressor and stopping or changing the NSAID. Because bacteria are not involved, antibiotics cant treat this type of ulcer. Kids who get a medicine-related ulcer should avoid NSAIDs and any medicines that contain ibuprofen or aspirin.

Unless a particular food is bothersome, most doctors dont recommend diet changes for kids with ulcers. A good diet with a variety of foods is essential to all kids growth and development.

Not all hematuria can be seen with the human eye. In fact, the most common type of hematuria-called microscopic hematuria-can only been seen by a health care expert under a microscope. In many cases, microscopic hematuria is spotted when a person has a urine test during a health exam.

When a person can see the blood in his or her urine, the condition is called gross hematuria. People with gross hematuria have urine that is pink, red or brown.

“There’s a common misconception that if you see blood in your urine once and then it goes away that you’re in the clear,” says Angela B. Smith, MD, Assistant Professor of Urology at the University of North Carolina School of Medicine in Chapel Hill. “But it’s important to seek care the very first time you see blood in the urine, so your doctor can confirm that it’s there and refer you to a urologist for an evaluation.”

In most cases, people with either type of hematuria do not have pain or any other signs or symptoms.

Uc Treatments And Urinary Changes

Sometimes, UC treatments can affect urinary habits. Corticosteroids such as prednisolone may cause urinary changes in people with UC. This medication usually causes someone to pee less, but it can also cause someone to pee more often.

Several members have noticed this effect. Has anybody ever experienced frequent, urgent urination whilst on prednisolone? asked one member. I am definitely peeing more often, but the urgency at which I need to get to the bathroom to have a pee is a bit worrying. Another member mentioned that they had experienced this effect. When I take prednisone or budesonide, I urinate much more frequently, which is a known side effect of steroids.

Using high doses of steroids or using steroids for a long time can sometimes lead to problems with the adrenal glands small organs that make hormones to regulate many body processes. Frequent urination can be a sign of this issue. Other symptoms include dizziness, a fast heart rate, irregular heartbeats, blurry vision, or feelings of weakness. Tell your doctor if you notice these symptoms while taking steroids.

Other Causes Of Peptic Ulcers

Other potential causes of peptic ulcers include:

  • Genetics: Many individuals with peptic ulcers have close relatives with the same problem, suggesting that genetic factors may be contributing.
  • Smoking: People who regularly smoke tobacco

A persons description of symptoms will often cause a doctor to suspect a peptic ulcer during a routine exam.

Tests that can confirm a diagnosis include:

  • a blood test to check for H. pylori, though a positive test does not always mean there is an active infection
  • a breath test, which uses a radioactive carbon atom to detect H. pylori
  • a stool antigen test to detect H. pylori in the feces
  • an upper gastrointestinal X-ray to identify ulcers

The most common tool doctors use for diagnosis is EsophagoGastroDuodenoscopy or upper endoscopy. It involves inserting a tube through the mouth to look for the presence of an ulcer in the stomach and upper intestine.

If the test detects an ulcer, the doctor may take a biopsy for examination under a microscope. A biopsy can test for H. pylori and look for evidence of cancer. A person may need to repeat an endoscopy a few months later to determine whether the ulcer is healing.

Treatment will focus on either lowering stomach acid levels so that the ulcer can heal or eradicating the H. pylori infection. The following are common treatment methods a doctor may recommend:

How Does H Pylori Cause Ulcers

Although many people naturally carry H. pylori, it is not clear why the bacteria only cause ulcers in some people. H. pylori spread through food and water. They live in the mucus that coats the lining of the stomach and duodenum, and they produce urease, an enzyme that neutralizes stomach acid by making it less acidic.

To compensate for this, the stomach produces more acid, and this irritates the stomach lining. The bacteria also weaken the defense system of the stomach and causes inflammation. Endoscopy as Treatment

Endoscopy is usually used to treat bleeding from visible vessels that are less than 2 mm in diameter. This approach also appears to be very effective at preventing rebleeding in patients whose ulcers are not bleeding, but who have high-risk features . Endoscopic treatment includes epinephrine injection, thermocoagulation, application of clips and banding procedures.

The following is a typical endoscopic treatment procedure:

Endoscopy is effective at controlling bleeding in most people who are good candidates for the procedure. Rebleeding occurs in about 10% to 20% of patients. If rebleeding occurs, a repeat endoscopy is effective in about 75% of patients. Those who fail to respond will need to have an angiogram with embolization or major abdominal surgery.
