While wound management is a key part of sacral ulcer management, treating patients holistically is the key to success. Apart from ischemia, other factors that impede normal healing include poor nutrition, infection, edema, persistent moisture, fecal and urinary soiling, and shearing forces. One can look for, prevent, or minimize each of these risk factors. Of course, the patient should be frequently repositioned to avoid further tissue damage and to promote healing.

When selecting a dressing, the wound should be kept moist but not contain excessive amounts of exudate. Review question

We reviewed the evidence about the effects of dressings and topical agents on pressure ulcer healing. There are many different dressings and topical agents available, and we wanted to find out which were the most effective.

Background

Pressure ulcers, also known as bedsores, decubitus ulcers and pressure injuries, are wounds involving the skin and sometimes the tissue that lies underneath. Pressure ulcers can be painful, may become infected and affect peopleâs quality of life. People at risk of developing pressure ulcers include those with limited mobility â such as older people and people with short-term or long-term medical conditions â and people with spinal cord injuries. In 2004 the total yearly cost of treating pressure ulcers in the UK was estimated as being GBP 1.4 to 2.1 billion, which was equivalent to 4% of the total National Health Service expenditure.

Study characteristics

In July 2016 we searched for randomised controlled trials looking at dressings and topical agents for treating pressure ulcers and that gave results for complete wound healing. We found 51 studies involving a total of 2947 people. Thirty-nine of these studies, involving 2127 people, gave results we could bring together in a network meta-analysis comparing 21 different treatments. Most participants in the trials were older people three of the 39 trials involved participants with spinal cord injuries.

The PU model of rats established by magnets and the schematic illustration of the model are shown in Supplementary Figure S1. To evaluate the success establishment of the PU model, PU was first compared with the acute wound in wound healing closure on a macroscopic scale, and the pathological changes of PU were observed after H& E staining . Different to the acute wound that kept shrinking until it healed completely without the necrotic tissue, PU exhibited a significantly slower wound closure upon wound size and was covered with the necrotic tissue during its regeneration period . Furthermore, wound healing closure rates at each time of the two groups are summarized and shown in Supplementary Figure S2B. The wound healing rate of PU was lower than that of the acute wound group at each observation time. And on day 14, the closure area of PU was significantly smaller than that of the acute wound group that had healed almost completely . The results of H& E staining showed that the dermal part of normal skin tissues was arranged in an orderly manner with complete hair follicles and other skin appendage structures without obvious inflammatory cell infiltration. In contrast, skin tissue structures of PU were not integrated with multiple inflammatory cells that appeared in the disordered granulation tissue. All these results indicate that the self-healing ability of PU wound is significantly much slower than that of acute wound.

What Are The Stages Of A Pressure Injury

There are four stages that describe the severity of the wound. These stages include:

  • Stage 1: This stage is discolored skin. The skin appears red in those with lighter skin tones and blue/purple in those with darker skin tones. The skin does not blanch when pressed with a finger.
  • Stage 2: This stage involves superficial damage of the skin. The top layer of skin is lost. It may also look like a blister. At this stage, the top layer of skin can repair itself.
  • Stage 3: This stage is a deeper wound. The wound is open, extending to the fatty layer of the skin, though muscles and bone are not showing.
  • Stage 4: This stage is the most severe. The wound extends down to the bone. The muscles and bone are prone to infection, which can be life-threatening.

Sensitivity Analysis By Risk Of Bias

The planned sensitivity analysis for risk of bias was to restrict the network to those studies at low or unclear risk of bias. Only 12 studies with 13 interventions remained and these formed three isolated loops.

Instead we conducted a sensitivity analysis which excluded studies that had high risk of bias for two or more domains we excluded seven studies from the joined network one further study was no longer joined into the network. This left 31 studies with 35 comparisons, including 18 interventions and 1513 participants .

The NMA results for interventions versus saline gauze are shown in Table 23 alongside the original data. There were only minor differences. The mean rank order was similar to the original data and the rankograms similarly indicated much imprecision.

Following Trauma Or Surgery

Persistent or recurrent epithelial defects may heal more rapidly following the application of a soft bandage lens. A small aqueous leak following surgery or trauma can often be sealed with such a lens if the anterior chamber is shallow or absent then a slightly flat lens will be needed, and this will have to be changed for a steeper lens as the chamber reforms and the cornea steepens.

Shoham et al. have successfully used a custom-made 17.5 mm diameter 78% water content bandage lens to arrest leakage from trabeculectomy filtration blebs. Corneal transplant problems such as loosening sutures or slippage of the donor disc are usually best dealt with by further surgery, for example suture removal and / or resuturing. In such situations, contact lenses are unlikely to have more than a temporary role .

In order for bed sores to heal, attention must be paid to the removing dead tissue and protecting the wound from infection causing bacteria. Dressings are usually applied to help the body heal itself. The type of dressing and the frequency with which it is to be changed is ordered by a physician with the application and changes carried out by nurses.

Many patients with bed sores suffer additional harm when the staff responsible for caring for them fails to follow medical orders with respect to the frequency with which dressings are to be changed. If dressings are not changed according to orders set forth by a physician, the healing of the bed sores may be delayed and perhaps become infected.

The most commonly used dressings used to treat bed sores include:

Absorptive Dressings: These dressings are either applied directly to the wound or on top of other primary dressings. Absorptive dressings are intended to remove the drainage from the bed sore that may impede healing. Most absorptive dressings are changed on a daily basis. However, excessive drainage from a bed sore may require more frequent dressing changes.

Pressure-induced skin and soft tissue injuries are often classified using the National Pressure Ulcer Advisory Panel staging system . Under this rubric, the wound should be staged to its deepest extent. This means selecting the highest number stage that accurately describes any part of the wound.

  • Stage 1 Pressure Injury: Non-blanchable erythema of intact skin

    Appendix 1 Pressure Ulcer Grading

    One of the most widely recognised systems for categorising pressure ulcers is that of the National Pressure Ulcer Advisory Panel . Their international classification recognises four categories or stages of pressure ulcer and two categories of unclassifiable pressure injury, in which wound depth and/or extent, or both, cannot be accurately determined unclassifiable pressure ulcers are generally severe and would be grouped clinically with Stage 3 or Stage 4 ulcers :

    The two additional categories of unclassifiable wounds are:

    • Unstageable/unclassified Obscured fullthickness skin and tissue loss: Fullthickness skin and tissue loss in which the extent of tissue damage within the ulcer cannot be confirmed because it is obscured by slough or eschar. If slough or eschar is removed, a Stage 3 or Stage 4 pressure injury will be revealed. Stable eschar on the heel or ischemic limb should not be softened or removed.

    Why It Is Important To Do This Review

    Pressure ulcer prevention and management is a significant burden to all healthcare systems. It is an internationally recognised patient safety problem and serves as a clinical indicator of the standard of care provided. Pressure ulcers are the second most reported incident that leads to patient harm in the health system, and are a significant source of suffering for patients and their care givers . Over recent decades significant investment has been placed in strategies aimed at pressure ulcer prevention. Treatment strategies for pressure ulcers can also be costly and complex, and there is a large range of wound care products available. Despite a growing amount of literature concerned with wound care interventions, relatively few research studies have used clinical trial methodology to evaluate clinical effectiveness. The complexity of suggested interventions, and range of options available suggests that the evidence requires evaluation and presentation to the clinician to assist with effective decision making. This review is part of a suite of reviews investigating the use of individual dressing types in the treatment of pressure ulcers. Each review will focus on a particular dressing type. These reviews will then be summarised in an overview of reviews which will draw together all existing Cochrane review evidence regarding the use of dressing treatments for pressure ulcers.

    The duration of followup ranged from 3 to 26 weeks, but the distribution was insufficient to allow modelling of time dependence in the network.

    Seven studies reported timetoevent data. We calculated the hazard ratio using the method and spreadsheet from Tierney 2007 one study reported the hazard ratio directly, adjusted for exudate level. The timetohealing data are shown in Analysis 3.1 and summary statistics for the timetohealing and the proportion healed are compared in Table 22 for the studies that report both healing outcomes.

    In the individual network, two studies in 95 participants suggested that the time to healing may have been quicker for hydrocolloid versus saline gauze there was no heterogeneity . One study in 24 participants suggested healing may have been quicker for collagenase ointment compared with hydrocolloid . In the other studies, the CI showed much uncertainty.

    There was some suggestion of a time dependent effect because there were qualitative and quantitative differences between the HR and the RR: for shorter studies , the HR gave a smaller effect than the RR, but for the medium and longer term studies the HR gave a larger effect than the RR, suggesting that wounds that heal do so relatively quickly.

    Spotlight On Aging: Pressure Sores

    Aging itself does not cause pressure sores. But it causes changes in tissues that make pressure sores more likely to develop. As people age, the outer layers of the skin thin. Many older people have less fat and muscle, which help absorb pressure. The number of blood vessels decreases, and blood vessels rupture more easily. All wounds, including pressure sores, heal more slowly.

    Certain risk factors make pressure sores more likely to develop in older people:

    Causes that contribute to the development of pressure sores include

    Pressure on skin, especially when over or between bony areas, reduces or cuts off blood flow to the skin. If blood flow is cut off for more than a few hours, the skin dies, beginning with its outer layer . The dead skin breaks down and an open sore develops. Most people do not develop pressure sores because they constantly shift position without thinking, even when they are asleep. However, some people cannot move normally and are therefore at greater risk of developing pressure sores. They include people who are paralyzed, comatose, very weak, sedated, or restrained. Paralyzed and comatose people are at particular risk because they also may be unable to move or feel pain .

    Friction can lead to or worsen pressure sores. Repeated friction may wear away the top layers of skin. Such skin friction may occur, for example, if people are pulled repeatedly across a bed.
