Best Probiotic Supplements for [Ulcerative Colitis]- Gut Surgeon Explains

Some of the conditions that might be helped by taking probiotics include:

Antibiotic associated diarrhoea

There’s good evidence that taking widely available types of probiotics alongside a course of antibiotics may help reduce the risk of getting diarrhoea.

They may also reduce the likelihood of getting a Clostridium difficile infection, which can have long-term serious consequences.

Successful strains include Lactobacillus rhamnosus GG, L. acidophilus , L. caseii and Saccharomyces boulardii .

Infectious diarrhoea

Probiotics can also reduce the severity and duration of infectious diarrhoea in children.

There’s no consensus on which strains of probiotics helps with IBS.

Irritable bowel syndrome

While there are studies that show some benefit from probiotics in reducing symptoms of IBS, there’s no consensus on which strains or dose.

The symptoms of IBS vary from person to person, so it’s considered unlikely that there’s a one size fits all treatment. That said, they could be worth a try discuss the options with your doctor.

Inflammatory bowel disease

Probiotics may be useful for treating pouchitis, a side effect of bowel surgery for people with ulcerative colitis.

However, the evidence for treatment of ulcerative colitis itself is inconclusive, with some studies suggesting a modest effect. Your doctor may be able to recommend products that could help.

There’s no evidence probiotics are useful for Crohn’s disease.

Eczema and other allergies

Coughs and colds

Inclusion And Exclusion Criteria

This study included clinical studies of UC with CHM intervention published in English and Chinese up to November 25, 2020. We included studies on subjects given the diagnosis of UC defined by clear diagnostic criteria or references, regardless of age, gender, course of disease and severity. The CHM interventions are typically administered as either single herbs or formulas , which may have been administered alone or in combination with other interventions of conventional Western Medicine or CAM. No limitation on their formulation, preparation, dosage, dosage form or route of administration. The control intervention could be active, conventional medicine, placebo, no treatment, or other CAM treatments, etc. There were no limitations in the assessed outcomes. We considered randomized controlled trials , quasi-randomized controlled trials, non-randomized controlled trials, observational clinical studies including the control group, casecontrol studies for inclusion. We excluded the following studies: repeated and withdrawal publications, studies with non-CHM interventions, comprehensive interventions focused on pharmacological treatment rather than CHM, plant extracts , clinical studies without a comparison or control group, study protocols, reviews, case series, case reports, abstracts or full-text reports not found, non-human studies, and non-English or non-Chinese language reports.

What About Prebiotics For Ulcerative Colitis

Think of probiotics as the flowers youre planting in the garden of your gut, and prebiotics as the fertilizer that encourages those flowers to grow.

Prebiotics are soluble fibers, the kind our bodies cant digest without the support of good gut bacteria to ferment it and break it down into useful short-chain fatty acids like butyrate, acetate, and propionate.

Prebiotics work together with probiotics to encourage the growth of good gut bacteria.

Probiotics May Have Benefits For Ulcerative Colitis But Not Crohns Disease

NEW YORK – A probiotic called VSL#3 may be effective in inducing remission in active ulcerative colitis , according to findings from a new systematic review and meta-analysis.

However, no benefits were found for probiotics in treating active Crohns disease or preventing relapse after remission.

VSL#3, a mixture of eight strains of lactic acid-producing bacteria, may be effective in inducing remission in active ulcerative colitis, corresponding author Dr. D. J. Gracie, of Leeds Gastroenterology Institute at the University of Leeds, in the UK, told Reuters Health by email.

However, in CD, use of probiotics is poorly described in a limited number of randomized controlled trials, and cannot be advocated, he cautioned.

Both of these types of inflammatory bowel disease are thought to result from interaction of genetic and environmental factors. The latter include changes in the intestinal microbiome, typically a relative increase in pro-inflammatory species and decrease in anti-inflammatory species.

The use of probiotics in gastrointestinal disease is well studied in IBD populations where there is an existing evidence base that supports their use, Dr. Gracie said. Given the high prevalence of functional symptoms in IBD, this was a logical systematic review and meta-analysis to undertake.

In those three studies, 91 of 162 patients who received VSL#3 failed to achieve remission compared with 118 of 157 of patients who received placebo .

The Deal With Therapy And Uc

Its more than just talking: Psychological therapies show good efficacy in reducing IBS symptom severity, according to a study in Psychology Research and Behavior Management. More specifically, cognitive behavioral therapy has been well-known for functional bowel disease for a long time, Dr. Poppers says. In the study, CBT was been tested rigorously in multiple randomized controlled trials and consistently demonstrated significant and durable effects on IBS symptoms and quality of life.

  • 2Hospital of Chengdu University of Traditional Chinese Medicine, Chengdu, China
  • 3Department of Pharmacy, Sichuan Provincial Orthopedic Hospital, Chengdu, China
  • 4Health Preservation and Rehabilitation College, Chengdu University of Traditional Chinese Medicine, Chengdu, China

Background: The combination of probiotics and traditional Chinese medicine is a prospective therapy for ulcerative colitis , and its efficacy and safety need to be urgently evaluated.

Objective: This study aims to comprehensively assess the efficacy and safety of probiotics combined with TCM for the treatment of UC.

Methods: The Pubmed, EMBASE, Cochrane library, China Academic Journals , Wan-fang database, Chinese biomedical literature service system , and Chinese Science and Technology Journals were searched. Subgroup analysis were designed in accordance with different control drugs, treatment courses, and types of probiotics. The Review Manager software was utilized for statistical analysis.

It is suggested that probiotics combined with TCM could effectively control clinical symptoms, inhibit intestinal inflammatory response, and finally slow down the disease progress and reduce the disease recurrence with less adverse events. The mixture of different probiotics used in conjunction with individually tailored TCM is a potential clinical strategy for UC.

What Should I Look For When Choosing A Probiotic

The Best Probiotic For Ulcerative Colitis

Other probiotics provide different types or strains of good bacteria in different amounts compared with VSL#3®. The number of good bacteria is measured in colony-forming units . Studies have shown that greater diversity of bacteria may provide better results. Studies have also shown that bacteria need to remain alive in the digestive tract in high amounts to have a positive impact. When choosing a probiotic, you should consider both the number of different strains and the total CFU count, which is usually measured in billions.

When The Strain Names Are Not Listed

Best of The VSL#3 Difference

The strain names arent listed for many probiotics on the market which makes it hard for consumers to make educated decisions about their purchases. A few like Culturelle, Jarrow and FloraStor do list strain names and hopefully many more brands will do the same in the future. Although some companies will disclose the strains used with a simple call, many in my personal experience do not and claim that they are not able to disclose it for proprietary reasons. And occasionally, when they do know the strain name, its often a poorly researched strain. My advice is to not purchase probiotics that dont list strains used.

Which Probiotic Should I Take

Unfortunately there isn’t one probiotic that works for all. It is very individual as to whether probiotics will make a difference to you and depends on a number of factors including which type of IBD you have.

Probiotics can be expensive and there are dozens available to buy. They are all of varying quality and some are bought from a common distributor and rebranded by the company selling them. Some brands of probiotic do have clinical research available into their finished products in relation to IBD, but most do not.

Due to the cost of probiotics its worth spending some time doing research into them – such as the claims they make and the clinical research behind their product – before buying them. You may also find the experiences of other people with Crohn’s disease and ulcerative colitis who use probiotics useful in making your choice.

The success you have with probiotics will be very individual and if you find that one product doesn’t work for you it doesn’t mean that none of them will. Unfortunately you may have to go through a process of trial and error to see if they are of benefit to you.

What are the different types of manufactured probiotics?

Manufactured probiotics available generally fall into three types:

Do They Help The Verdict On Probiotics And Ulcerative Colitis

Multiple studies have concluded that certain strains of bacteria can help prevent a relapse of ulcerative colitis symptoms.

The right combination of strains, such as those found in VSL#3, might even help people go into remission during a flare-up.

More research is needed though, because the word probiotics refers to countless species of bacteria. Evaluating each of them and their myriad combinations requires a great deal of testing to figure out how best to deploy them for each unique case of IBD.

How To Take Probiotics

When you take probiotics for UC, there are two key things to know.

  • You’ll probably need to take them for a while. Your stomach acid kills bacteria, whether good or bad. Experts suggest that you take the probiotics for at least 7 to 10 days to get enough good bacteria. Studies have treated people with probiotics for up to 6 to 8 weeks. You need millions and millions — possibly billions — of good bacteria to start relieving UC symptoms.
  • To keep up the benefits, you have to keep taking the probiotics. If you stop, the balance of bacteria in your colon will change and you may get a flare-up. Talk to your doctor about a long-term probiotic plan.

  • Taking them by mouth may not be best. Although it may not seem pleasant, there may be some advantages to taking probiotics rectally. A smaller dose may be possible because the probiotics skip the stomach acid. This lets more of the good bacteria reach the intestines. But for obvious reasons, most people find it easier to just swallow a pill.

The disease can cause loss of appetite, making it difficult for people to eat enough food to get sufficient amounts of recommended vitamins and minerals. During flare-ups, it may be necessary to temporarily avoid certain foods. And an inflamed colona primary symptoms of ulcerative colitissimply is not able to absorb nutrients as well as a healthy colon, which means people with ulcerative colitis are at risk for nutritional deficiencies. Symptoms Of Ulcerative Colitis

Symptoms of ulcerative colitis vary depending on the severity of inflammation and which segment of the large intestine is affected. Possible symptoms of ulcerative colitis include:

    • Abdominal cramping and pain
    • Profuse diarrhea, possibly with blood
    • Difficulty having bowel movements in spite of urge
    • Weight loss

    A Final Note On Probiotics

You might be wondering- how do you know if probiotics are working? It isnt easy to determine this based on symptoms alone. As Ive mentioned in other articles on the types of remission, symptoms alone arent always the best indicator of inflammation and our IBD.

We can be symptom free and have ongoing disease activity and inflammation AND we can also have irritation symptoms like gas, bloating, diarrhea and have no inflammation present.

The research on probiotics and IBD can vary quite a bit on how long it takes to see results with some studies showing symptoms improving 6 months to a year after taking probiotics.

Its important to realize that most of the time its not one thing that makes a difference for IBD- its a layered approach of bringing in many evidence based strategies with probiotics having the potential to be one of them.

What Are The Prescription Medications For Uc

    Best of The VSL#3 Difference

Medication for UC is designed to suppress your immune systems abnormal inflammatory responsewhich is causing symptoms. When inflammation is suppressed, it offers relief from common symptoms .

There are 5 main categories of medications used to treat inflammatory bowel disease like UC.

Given either orally or rectally, these drugs work to decrease inflammation in the lining of the intestines and are usually used to treat mild to moderate UC symptoms. Sulfasalazine and mesalamine are examples of 5-ASA medications.

Metronidazole, ciprofloxacin, and other antibiotics may be used when infections occur, or to treat complications of ulcerative colitis.

These suppress the immune system to reduce inflammation by targeting a specific pathway. Certain biologics work by targeting specific inflammatory proteins that play a role in inflammation called cytokines. Others work by preventing certain white blood cells from getting into inflamed tissues.

Prednisone, prednisolone, and budesonide are included in this type of medication. They affect the bodys ability to initiate and maintain an inflammatory processkeeping the immune system in check. While effective for short-term flare-ups, theyre not recommended for long-term use.

These modify the bodys immune system activity to stop it from causing ongoing inflammation. These drugs , and methotrexate) are usually used to maintain remission or for people who have only responded to steroids.
