What Is Bottoming Out

Generally, when an adjustable bed is inclined at the head or foot, the force of gravity naturally shifts the users weight to the center of the bed. To prevent the user’s midsection from bottoming outbeing so heavy that it overwhelms the mattress to the point that their bottom rests on the bed deckalternating pressure systems will sense when the mattress is in an inclined position and force needed air to the middle of the mattress. Mattress models vary, but 10- to 15-degrees of incline is generally the threshold at which the bottoming out prevention mode is triggered. Inclining mattresses beyond this point is perfectly fine for the system. But it is important to note that while in this bottoming out prevention mode, the alternating pressure function will be automatically put in standby as the mattress enters Static mode to redirect the needed air. How often a user rests in an inclined position, the amount of incline and duration will depend on the pressure prevention program developed by your medical professional team.

How A Low Air Loss Mattress Can Help Keep Patients Wound

For patients suffering from pressure ulcers or patients at a high risk of developing pressure ulcers, low air loss mattresses can provide an essential form of treatment proven to keep patients at home in their beds and away from the hospital.

For most of us, our bed is a place of rest and reprieve at the end of each day. Itâs the spot where we can let go of the stress of our to-do list or the big project thatâs due next week and drift off into a sweet slumber. But for patients confined to a bed due to illness or limited mobility, the bed can be a source of anxiety and pain, particularly when pressure wounds arise.

Depending on the severity of the wound, patients suffering from pressure wounds can experience everything from mild discomfort to extreme pain, but with proper management , healthcare providers can dramatically reduce the frequency of these unwanted occurrences and help treat them when they arise, ensuring patients get the rest they need on the road to recovery and optimum health.

A Quick Recap Of Pressure Wounds

Pressure wounds are soft tissue injuries resulting from constant pressure that leads to lack of oxygen and other nutrients, causing skin cells to die. These wounds are most often the result of sustained pressure, friction, or shear and can lead to many complications like sepsis, cellulitis, bone and joint infection, and in some cases, even cancer. For a more in-depth look at the common causes of pressure, wounds check out this post.

Development Of The Bedsores

Pressure ulcers can develop in multiple ways however, it’s always through prolonged pressure of some type. When they arise, it’s critical to assess how serious the bedsore is and take appropriate care to treat it. If the damage is already moderate to advanced, a dynamic or bubble-style mattress where you can deflate cells near the sore should work best.

How To Prevent Pressure Ulcers

Maintaining a watchful eye on a loved one’s skin and keeping them comfortable with frequent adjustments is key for preventing these dangerous and painful sores. If someone needs to rest in bed at all times, shift their position often to reduce the amount of pressure applied to one spot.

Additionally, make sure they’re kept dry. Moisture can lead to ulcers, so using a moisture barrier cream to protect against wetness can help keep skin clean and dry. You may need to keep their clothes and bedding switched out regularly.

If your loved one uses a wheelchair predominantly, foams can relieve pressure from sitting in the same position all day. Shifting an immobile person is a lot of tough work, but ultimately should keep them happier and healthier.

Group 2 pressure-reducing support surfaces, which can include alternating-pressure mattresses, low-air-loss mattresses and mattress overlays, are indicated for use as a prevention or treatment for pressure ulcers, bedsores and other types of skin tissue breakdown. Each pressure-relieving mattress featured in this section is designed to improve patient comfort. Pressure-reducing mattresses redistribute a patients weight so as to relieve pressure points. An alternating air mattress, which is a specific type of medical air mattress, contains air cells that are continually inflated and deflated, which relieves pressure points and promotes better circulation.

What Does The Cochrane Overview Tell Us That We Didnt Know Before

KosmoCare Anti Decubitus Air Mattress MM1 for Prevention of Bed/Pressure Sores.

Overall, we concluded that it is unclear which support surface is best in preventing and treating pressure ulcers.

Unfortunately, we cant trust most of the results from the two networks. This is mainly because many of the original trials were poorly conducted, small, and trial data in the network could not produce precise results.

However we found some new evidence:

  • A reactive air surface may be better than a foam surface or overlay for preventing and healing pressure ulcers. Foam mattresses are recommended in the 2014 NICE clinical guideline and are routinely used in practice.
  • Alternating pressure air surfaces may be better than foam surfaces for preventing pressure ulcers. Alternating pressure air surfaces probably cost less, overall, than foam surfaces in relation to their benefit in preventing ulcers.
  • Reactive gel surfaces may be better than foam surfaces for preventing pressure ulcers. This type of surface is particularly relevant for people undergoing surgery that lasts several hours.

Current Recommendations And Practice For Preventing And Treating Pressure Ulcers

The 2014 NICE clinical guideline in the UK Pressure ulcers: prevention and management, has recommended using foam mattresses for preventing pressure ulcers in adults at risk of having a pressure ulcer, and for treating pressure ulcers.

However, other types of support surfaces such as reactive air mattresses or overlays, and alternating pressure air mattresses or overlays, are also commonly used.

The NICE guideline does not contain evidence on how support surfaces compare, or evidence on which support surface is the most effective. This leaves decision-makers with a confusing array of devices to choose from but evidence uncertainty.

How Does A Pressure Mattress Work

Pressure mattresses are designed for those with pressure ulcers . Pressure sores occur when an individual spends prolonged periods of time in the same position and typically occurs in those with restricted mobility and/or poor skin integrity. It is crucial to get the right type of pressure mattress for the right grade of bed sore in order to maximise patient comfort.

This blog will explain what a pressure mattress is and how it differs from a regular mattress. It will also show how a pressure sore mattress works and how they are effective for increasing comfort for those suffering from bed sores.

Results Of The Review

Air Mattress For Pressure Ulcer Prevention

Thirty-five studies were reported in the review: 20 randomised controlled trials 13 experimental studies one prospective study and one retrospective study. Sample size ranged from 10 to 1,971 patients.

Study quality: 18 randomised controlled trials either had insufficient power or did not estimate the required sample size 11 randomised controlled trials had unclear or inadequate randomisation and four randomised controlled trials did not report baseline comparability of treatment groups. The sample size of laboratory studies was small and nine such studies involved only healthy people.

Randomised controlled trials assessing effectiveness

Experimental studies assessing contact interface pressure and blood perfusion

Two studies compared alternating pressure air mattresses versus standard hospital mattress one reported significantly lower contact interface pressure with the alternating pressure air overlays and the other reported significantly higher tcPO2 with the alternating pressure air overlays. Four studies compared alternating pressure air mattresses with different constant low-pressure surfaces and seven studies compared different types of alternating pressure air mattresses findings were inconsistent.

Mechanical reliability and user errors were also reported in the review.

What Is A Pressure Relief Mattress

A pressure relief mattress is designed to increase comfort for an individual suffering from pressure ulcers. The mattress provides high levels of support for the head and body to relieve any mounting stress on pressure points.

Unlike a regular mattress, pressure relief mattresses have carefully designed air pockets, specifically designed to place less pressure on the main pressure points in the body.

There are three main types of pressure relief mattresses to consider.

What Are Pressure Ulcers

Also called pressure sores, scabs or sores. A pressure ulcer is any skin lesion that occurs anywhere in the body suffers a prolonged pressure on a flat duro.

Esta pressure not necessarily has to be intense and is independent of the position in which the patient remains

The skin area affected by unrelieved pressure may deteriorate but also the tissue beneath it is -fat, muscle, bone-.

Pressure ulcers represent a major health care problem due to its prevalence and impact on health and quality of life of patients. In addition.

It is currently one of the most important chapters of the work of nurses and cause enormous costs to the health system.

The prevalence of pressure ulcers in hospitalized population shows a wide variability between 3.5% and 29%.

The overall incidence in hospitals in Spain is approaching 8%.

How To Carry Out A Risk Assessment

NICE recommend that the most effective way of carrying out a pressure ulcer risk assessment, is to include a validated risk assessment tool to support clinical judgement.

Under these set of guidelines, it also includes a third factor to consider.

To enhance the prevention and treatment of pressure ulcers, care providers must also reassess pressure ulcer risk if there is a change in clinical status.

For example, this could include the worsening of an underlying health condition or a change in mobility.

How Can A Mattress Help In Pressure Ulcer Prevention

A mattress can help prevent bedsores from appearing by evenly distributing the weight so that no one spot receives an overly concentrated amount of pressure. This is often challenging with individuals who are bedridden or immobile, however, frequent adjustments are critical for prevention.

Various types of mattresses work well for preventing these ulcers.

Air loss cushions work by simulating a weightless sensation, relieving pressure from the afflicted area. Dynamic beds work by inflating bubble cells for support and deflating ones near the infected region for relief.

Static mattress work by using modern foams to distribute weight and reduce the chance of bedsores and prolonged tension. Finally, hybrid mattresses are a combination of a static and a dynamic mattress, using features beneficial to both designs, and work well for high-risk individuals.

What Is A Low Air Loss Mattress

Reducing heat and moisture is an important component of any pressure ulcer prevention program. Low air loss is a feature that provides a flow of around the surface of the mattress to assist in managing the heat and humidity of the skin. Tiny laser micro air holes in the top surface of the mattress blow out air which reduces skin interface pressure and wicks away moisture. This keeps the skin dry and helps with the treatment and prevention of pressure ulcers. There are two types of Low Air Loss Mattresses: On-Demand Low Air Loss, and True Low Air Loss. Mattresses with On-Demand Low Air Loss have a Low Air Loss Mode that can be turned off and on. In comparison, a True Low Air Mattress runs the feature continuously.

Sleep An Essential Component Of Optimal Health

As we learned in this post on the importance of gel mattress overlays, the NIHâs National Heart, Lung and Blood Institute says, âsleep plays a vital role in good health and well-being.â Sleep is involved in many essential processes, including the healing and repair of blood vessels , so taking care to create the best possible sleep and resting environment for patients should be a top priority for healthcare providers.
