I encourage my patients to eat the greatest variety of plant-based foods they can comfortably tolerate, which may look different whether you are in a flare versus remission, says Freuman.

In other words, trial and error is key to figuring out what works best for you.

Supplements And Other Lifestyle Changes To Try

Besides diet and medicine, there are a few other therapies that could be helpful for people with ulcerative colitis.

Dr. Singh suggests focusing on adequate sleep and reducing stress to prevent flares. One of the most common causes of flare can be stress, so stress reduction is probably the best medicine I talk about when I see patients, he says.

There is also limited research surrounding the use of certain supplements and probiotics as a therapy for IBD, but more evidence is needed before recommendations are made12,13.

Challenges In Creating Evidence

Although there are diet intervention trials that show promise in maintaining remission, their efficacy remains in question. The short duration of the interventions , the lack of a proper control group in some instances, and the small sample sizes make it very challenging for clinicians to draw firm conclusions from existing data. There is an overall lack of objective clinical and endoscopic disease markers. For example, many studies are completed retrospectively and they rely on patient questionnaires regarding disease symptoms, such as pain and stool frequency. Well-designed clinical trials in IBD are urgently required to define the precise role of each of these diets in the prevention or management of IBD. Up until now, the role of diet in IBD is highly undermined by lay and anecdotal reports without sufficient scientific proof.

High quality diet intervention studies for the treatment of IBD need to include the following: quantification of baseline intake of the habitual diet monitoring of diet adherence through food recalls large prospective, control trials over a longer-term use of a control diet to determine the specificity of observed effects to the intervention use of a variety of endpoints to monitor response to diet interventions and consider the use of IBD animal models to discover the mechanisms of pathogenesis.

Protein Intake As A Risk Factor For Ibd

A large prospective cohort study completed over a 10.5-year period found that high protein intake, specifically animal protein was positively associated with an increased risk of IBD . A systematic review reported an association with high total protein intake with the development of UC and CD . High protein intake was associated with a 3.3-fold increased risk of IBD, suggesting a diet high in animal protein is a major risk factor for the development of IBD.

Study Design And Participants

In the present cross-sectional study, the diet adequacy and food avoidance habits of UC patients in the UK were investigated. The study was conducted according to the guidelines laid down in the Declaration of Helsinki, and all procedures involving human subjects were approved by the ethics committee of a UK Higher Education Institution. Written informed consent was obtained from all participants.

A total of ninety-seven UC patients were recruited across the UK from the National Association for Colitis and Crohn’s Disease registers by e-mail, posts in UC forums and social media sites. The inclusion criteria were as follows: aged 1865 years and no history of UC-related surgery.

Initially, four participants participated in a pilot study to ensure that the questionnaire and 24 h dietary recalls were readable and not distressing.

What Foods Should I Avoid

Certain foods can exacerbate UC flares, but everyone has different trigger foods. For some, it might be a juicy burger and for others, it could be their morning latte.

In general, inflammatory foods, like fast food, processed food, alcohol, and sugary drinks contribute to the development of flares, says Dr. Singh. Freuman adds that saturated fat, specifically, can be an issue for certain people. Foods that contain significant amounts of saturated fat include:

  • Coconut oil
  • Whole-milk dairy, such as cheese, cream, butter, and full-fat yogurt
  • Fried foods
  • Red meat

Lactose is the sugar present in milk and dairy products, and lactase is the enzyme people need to break down those sugars during digestion. If someone is lactose-intolerant, it means they dont produce enough lactase to break down the sugars, which can cause cramps, bloating, diarrhea, and gas.

To complicate matters, UC is sometimes coupled with lactose intolerance. But lactose intolerant dairy lovers, take heart: Its often possible to still consume dairy with lower lactose content, like cottage cheese and yogurt. Because these products contain live cultures that produce their own lactase, your body doesnt have to do all the work to break down lactose5.

Thats a win-win, since dairy foods provide important nutrients, like calcium and vitamin D, to your diet. In fact, avoiding them completely is not recommended unless 100% necessary.
