Vaccinations While Taking Steroids

Its not safe to have live vaccines while taking high doses of steroids or for a longer period of time because steroids weaken your immune system. If this is the case, youll have to wait 3 months after you stop taking steroids before having a live vaccine. Speak to your IBD team to make sure your vaccinations are up to date before you start taking steroids, or if youre planning to travel. If youve recently had a live vaccine, you should wait least 3 weeks before starting steroids.In the UK, live vaccines include:

  • BCG
  • Measles, mumps and rubella
  • Yellow fever
  • Rotavirus
  • Flu nasal spray

Everyone with Crohns or Colitis should have the annual flu jab this is not a live vaccine and is safe to have while taking steroids.

If someone in your household is due to have a live vaccine, ask your IBD team if you need to take any special precautions.

Your IBD team will carefully guide you on this.Its very important not to miss a dose or suddenly stop taking steroids. While youre taking steroid medicines, you stop making enough of your own steroids which are important in keeping your body functioning. A sudden withdrawal from steroid medication may cause secondary adrenal insufficiency, a sharp fall in blood pressure and affect blood sugar levels. Youll need to undergo a gradual reduction of your steroid dose to give your body time to start making its own steroids again.Some symptoms of secondary adrenal insufficiency are similar to those of Crohns and Colitis, and include:

    Its key for patients to understand and spend some time with their doctor, learning more about the disease, the disease process, and the goal of the treatment, says Dr. Ali.

    Even if you dont need to change your treatment altogether, there are some ways to optimize your regimen. Take medications regularly as prescribed, says Ashwin Ananthakrishnan, MBBS, MPH, an associate professor of medicine at Massachusetts General Hospital in Boston. Check in periodically with your doctors for lab work, stool tests, and colonoscopies.

    Ananthakrishnan also stresses the importance of regular physical activity, adequate sleep, stress management, and a healthy diet that includes vegetables and fruits and limits sugars, processed foods, and unhealthy fats.

    Probiotics may modestly reduce disease activity in active disease, but do not increase remission rates.34 However, Lactobacillus GG and Escherichia coli Nissle 1917 have been shown to be as effective as 5-ASA for maintenance therapy.16,17 Acupuncture has shown a small symptomatic benefit in active disease.35 Wheatgrass has also shown some effectiveness in reducing symptoms of active disease.36

    Oral Prednisolone For Crohn’s Disease And Ulcerative Colitis

    You will usually take prednisolone tablets, gastro-release tablets and solution once a day, in the morning with breakfast. This means it will be less likely to upset your stomach or affect your sleep.

    You can swallow prednisolone tablets with water – you will probably have a few to swallow at a time. Gastro-resist tablets should always be swallowed whole and not chewed – they are specially coated to stop them dissolving in your stomach, and chewing or breaking them could stop them working properly. You should also avoid taking indigestion medicines for two hours before and two hours after taking gastro-release tablets.

    Prednisolone liquid solution can be measured out using a syringe and bottle adapter, in the amount prescribed by your IBD team. You should take it in the morning with or after food.

    Oral prednisolone is often prescribed as an eight week course of treatment, and your IBD team may ask you to reduce your dose gradually towards the end of your treatment. Remember it is important you dont suddenly stop taking prednisolone, even if you feel better, as this could make you extremely unwell.

    Dose Optimization Of Certolizumab Pegol And Golimumab

    Ulcerative Colitis/Prednisone Update

    Although assays are not commercially available to assess drug levels or antibodies to certolizumab pegol or golimumab, clinical trial data for both agents suggest that antibodies can form to both drugs, and the incidence of antibody formation to the drugs decreases with the use of immunomodulators. Dose intensification was also allowed in the MUSIC trial, discussed below, and may be an option in patients who are not responding to standard dosing of certolizumab pegol.

    In a trial assessing golimumab maintenance therapy for patients with UC, the incidence of antibodies through week 54 was 2.9% . The rate was lower for patients who received concomitant immunomodulators vs patients who were on golimumab monotherapy .
